How do I access my 1098-T?

1. From the FSU Home Page, Go to: Current Student (or use the link below)
2. Select: Banner Login
3. Login with USER ID and PIN
4. Go to: Student Services and Financial Aid
5. Go to: Student Account
6. Go to: Tax Notification
7. Enter Tax Year: 2011
8. Click: Submit
9. Review for accuracy and Print for your records.

Instructions for Student:

An eligible educational institution , such as a college or university, in which you are enrolled who makes reimbursements or refunds of qualified tuition, fees, scholarships, and grants, are required to furnish this information to IRS on a 1098-T form for the calendar year. Instructions on how to print for your records are located in Banner under the Tax Notification form.
