The assessment report documents are available in two formats. You can choose to complete and print out the PDF fillable document, or you can save a copy of your report in Microsoft Word. They are both identical so you can use the version that is most convenient for you. Please turn in a copy of the document to your Assistant Department Chair or the faculty member assigned to work with assessment in your Department.

You can fill in the following fields: Core Learning Outcomes, Evidence, # and % of students completing the course who have satisfactorily achieved the outcomes), and Use of Results. The sections at the bottom of the form regarding CLOs (Course Learning Outcomes) from individual courses are optional. Only the PLOs (Program Learning Outcomes) are required.
