ENTRANCE CONFERENCE


If you have a Federal Perkins Loan, please complete the form below and the Federal Perkins Loan Entrance Counseling session. Please bring this form along with the confirmation page from your Federal Perkins Loan Counseling session to our office. This will shorten your Federal Perkins Loan entrance conference time.

If Federal Perkins Loans are a part of your Financial Aid Award Package, you must complete the "Federal Perkins Loan Entrance Conference" before the funds can be transferred to your student account in the Business Office.

NOTE: This Federal Perkins Loan Entrance Conference is in addition to the one that is required by the Financial Aid Office. 

Federal regulations require the University to provide a loan conference to every borrower, whether you are an undergraduate or a graduate student. The goal is to emphasize the importance of loan repayment and the consequences of defaulting on a student loan. The conference also reviews and explains your rights and responsibilities as a borrower as well as advises you of the rights and responsibilities of the University (the lender). This information must be provided to you before funds are transferred to your student account. 

You can complete the Federal Perkins Loan Entrance Counseling on the web at Mapping Your Future. Follow the instructions on the website. When you complete the session, you will receive a confirmation number. Please print out the confirmation information and bring it along with your FSU student ID, driver's license and social security card to the Federal Perkins Loan Office, C. J. Barber Building, Room 139 in order to sign your promissory note and  finalize receipt of your Federal Perkins Loan.
