Instructions 

1. Application MUST be typed and all areas, completed where  appropriate (inserting "see attached" is not acceptable).
2. Verifications must be attached for listed refereed journal publications.
3. A vitae is not a substitute for the application.
4. Attach additional sheets If necessary. 
5. Please submit completed applications with required signatures and verification information to The Graduate School, Attention: Graduate Council by one of the following methods:
   1. Hand Deliver to The Graduate School in the Continuing Education Building, Room 102
   2. Forward by FSU interoffice mail, or
   3. U.S. Postal Service
