To open PDF files, you must have a PDF reader program such as Adobe Acrobat Reader. You must have the full version of Adobe Acrobat, not Adobe Reader, in order to save your information. If you have Adobe Acrobat Full Version, not Adobe Reader, and choose to save your form input, you must save the form to your drive.

Microsoft Word and Excel

To download the Microsoft Word or Excel forms, right-click on the form's link and select "Save Target As...". When opening the Microsoft files, a Network Authentication login message may pop up. If this is the case, just click "Cancel" to continue loading the document and save the form to your drive before entering information.
