statement below:
this certifies that i know and understand that intramural participation involves inherent risks, such as, but not limited to, scrapes, cuts, or abrasions from falling , muscle or ligament strains from running and jumping, and broken bones or bruises from accidental collisions, and that even when safety precautions are utilized, injuries can occur. i understand that if i experience unusual pain or physical discomfort during participation in this sport, i will stop playing and inform the supervisor of my symptoms. i can claim to the best of my knowledge that i do not have any medical/physical disability that will prevent my safe participation in this program. i further certify that i will always wear any required equipment while playing and abide by all rules, regulations, and policies applicable to this program.
It is strongly recommended that all participants have satisfactory health status and appropriate accident insurance coverage for any injury which might occur during intramural participation.
