If you were admitted or re-admitted to the university prior to fall 2004, please contact this office for a copy of the appropriate degree evaluation plan. 

• 2006-2008 Catalogue Years: Use these evaluation forms if you were admitted for re-admitted to the university fall 2006 through summer II 2008
• 2004-06 Catalogue Years: Use these evaluation forms if you were admitted or re-admitted to the university fall 2004 through summer II 2006
