North Carolina State law requires that all students present documented proof of specific immunizations. The required immunizations are listed on the medical history/immunizations form required of all students prior to registration. Medical history/immunization forms are available at the Spaulding Infirmary. All medical information is confidential and is not divulged without the written consent of the student.
All new students, transfer students, returning former FSU students, students taking more than four hours on campus before 5:00p.m., and students (graduate and undergraduate) applying for admission to the Teacher Education Program must complete the medical/immunization requirements.* The immunizations are required by the state of North Carolina and must be physician-certified or verified by a clinic stamp before registering at FSU. Failure to comply with the state immunization documentation requires administrative withdrawal from the University.
*Students enrolled for four credit hours or less and residing off campus do not have to comply with immunization requirements. The same exemption also applies to students attending evening (starting at 5:00 p.m.) and weekend classes only, and students taking off campus classes for the duration of their degree program.
The University is required by law to withdraw any student who does not provide the appropriate immunization records. This administrative withdrawal can result in loss of grades and/or credit for your courses.
In non-emergency situations, University Police will provide transportation to Student Health Services. The Rescue Squad (911) should be called for students who appear to be seriously ill or injured. The student is responsible for charges or bills for Rescue Squad or ambulance services. Student Health Services is de- signed to serve students in the Spaulding Infirmary and is not prepared to care for students elsewhere. The Student Health Services staff does not make building calls.
In order to give satisfactory service to each student, the Student Health Services staff desires the cooperation of each student in adhering to health services policies. Any student who has a medical problem or question is invited to consult the Student Health Services staff.
