If you are applying to a graduate school, professional program, or for employment that requires a completed Dean's Certification that documents your student conduct/disciplinary record during your period of attendance at Fayetteville State University, please send or fax your certification form and signed/authorization to:

In addition, students need only to bring the appropriate forms to the Division of Student Affairs for completion. If the form does not include a "waiver" section and signature - where the student agrees and consents to the office's release of their personal information - the student must include a signed letter indicating his or her request and consent. Please allow 7 to 10 days for turn around on this paperwork. Double check that you have fully filled out and signed all applicable locations.
