Register for New Student Orientation

You are required to complete the AFROTC online application

In addition, below are the required documents to complete your Det 607 application package:

• Applicant Admissions Guide
• Cadet Orientation Acknowledgment
• AF Form 2030, USAF Drug & Alcohol Abuse Certification
• DD Form 93, Record of Emergency Data
• DD Form 2005, Privacy Act Statement - Health Care Records
• DD Form 2983, Recruiter/Trainer Prohibited Activities Acknowledgement
• AFROTC Form 28, Sports Physical (30 days prior to the start of the term) (Save to desktop)
• AF Form 4428, Tattoo/Brand/Body Marking Screening/Verification (Save to desktop)
• Drug Testing Policy
• Request and Consent for Release of Student Records
• SAT/ACT Verification
• Original birth certificate (we will copy and return original to you)
• Original Social Security Card (we will copy and return original to you)
• Original proof of citizenship (i.e. naturalization certificate if not born as a US Citizen) (we will copy and return original to you)
• Selective Service Number (males only) (need to register at age 18, you may confirm and print verification at www.sss.gov/Home/Verification)
• Current Immunization records (we will copy and return original to you)
• Unofficial Transcripts (for all college credit courses taken)
• Unofficial SAT (original score notice or high school transcript with posted scores; you can also pull from sat.collegeboard.org/home)
• Unofficial ACT (www.actstudent.org/)
• DD Form 214 copy 4 (if prior service)
• JROTC Certificate of completion or letter from school including total number of years completed (if applicable)
• Civil Air Patrol Award(s) (if applicable)
• Eagle Scout Award (if applicable)
• Private Pilot License (if applicable)
