Dining Accommodation Request Documentation Requirements
Fayetteville State University Accessibility Services reviews dining accommodation requests to determine eligibility and appropriate supports for students with accessibility barriers, ensuring full access to campus dining programs. To request accommodations, students must submit documentation that explains how a condition affects their access to meals; food allergies, special diets, etc. Documentation should clearly describe the functional impact of the condition and include recommended accommodations to support full participation.
Documentation Must:
- Be dated within the past 5 years.
- Include a medical diagnosis or health condition relevant to dining needs (e.g., allergies, gastrointestinal conditions, mobility limitations, chronic illness).
- Identify substantial functional limitations resulting from the condition, including severity, frequency, and duration, and describe how these affect access to meals or campus dining.
- Recommend specific accommodations that reduce or remove the barriers and support full participation in dining services (e.g., modified meals, meal delivery options, flexible access, or assistance).
Documentation Format:
- Must be on official letterhead and include:
- Provider’s legible name, credentials, contact information, and license number(s)
- Professional or electronic signature
Acceptable Providers:
- Licensed Medical Professionals
- Dietitians or Nutrition Specialists (for dietary needs)
- Psychiatrists or Psychologists (if mental health impacts dining or eating)
- Clinical Mental Health Counselors or Clinical Social Workers (if applicable for mental health barriers affecting nutrition)
Note: Documentation that does not include the functional impact or specific recommendations for dining accommodations may be returned in draft form for completion. All documentation must be uploaded through the AIMs portal to enter the review process.
