Temporary & Medical Accommodation Request Documentation Requirements
Fayetteville State University Accessibility Services provides temporary accommodations for students who experience short-term or situational accessibility barriers, including medical conditions that are expected to substantially impact functioning for a limited period. Temporary accommodations ensure students have equitable access to university programs, services, and housing while their condition is active.
Please ensure that all documentation for temporary accommodations includes a definite or official end date.
Do not submit absence verification requests via email or the AIMs portal. For guidance, please see → Absence Verification Process under the Faculty tab. Absence Verification requests submitted to Accessibility Services, via email or uploaded in AIMs will be returned.
Documentation submitted for temporary accommodations must:
- Be dated within the past 5 years.
- Include a medical diagnosis or diagnoses, if applicable.
- Clearly describe the functional limitation(s) caused by the condition, including severity, frequency, and duration of symptoms.
- Recommend specific accommodations that reduce or address these limitations.
- Include an expected end date for the temporary accommodation.
All documentation, including diagnostic assessments or medical reports, must be on official letterhead and include:
- Provider’s legible name, contact information, email address, and license number(s)
- Professional or electronic signature
Acceptable providers include licensed:
- Medical Professionals
- Psychiatrists or Psychologists
- Clinical Mental Health Counselors
- Clinical Social Workers
