Temporary & Medical Accommodation Request Documentation Requirements 

Fayetteville State University Accessibility Services provides temporary accommodations for students who experience short-term or situational accessibility barriers, including medical conditions that are expected to substantially impact functioning for a limited period. Temporary accommodations ensure students have equitable access to university programs, services, and housing while their condition is active. 

Please ensure that all documentation for temporary accommodations includes a definite or official end date.

DISCLAIMER: As of February 20, 2026, Accessibility Services no longer processes absence verification requests. Requests related to absences or assignments must be handled directly with your instructor.

 

Do not submit absence verification requests via email or the AIMs portal. For guidance, please see → Absence Verification Process under the Faculty tab. Absence Verification requests submitted to Accessibility Services, via email or uploaded in AIMs will be returned.
 

Documentation submitted for temporary accommodations must:

  • Be dated within the past 5 years.
  • Include a medical diagnosis or diagnoses, if applicable.
  • Clearly describe the functional limitation(s) caused by the condition, including severity, frequency, and duration of symptoms.
  • Recommend specific accommodations that reduce or address these limitations.
  • Include an expected end date for the temporary accommodation.

All documentation, including diagnostic assessments or medical reports, must be on official letterhead and include:

  • Provider’s legible name, contact information, email address, and license number(s)
  • Professional or electronic signature

Acceptable providers include licensed:

  • Medical Professionals
  • Psychiatrists or Psychologists
  • Clinical Mental Health Counselors
  • Clinical Social Workers
