Absence Verification Process

When students miss class due to a documented reason, students can submit the official documentation to the Counseling and Personal Development Center (CPDC).  The CPDC will send out confidential email notifications to your professors for documented* missed days:

  • Medical emergencies 
  • Doctor’s appointments 
  • Death in the family (covers date of death and date of funeral)
  • Court dates 
  • Military related obligations
Disclaimer: The Counseling and Personal Development Center does not excuse absences. The CPDC only verifies that documentation was received. Professors retain full discretion to determine whether an absence will be excused.

Examples of Accepted Documentation:

  • Doctor’s note on official letterhead with appropriate signature and credentials
  • Hospital discharge paperwork including specific dates and signature
  • Obituary notice
  • Court orders
  • Military orders

The Absence Verification Process includes the CPDC team confirming that the submitted documentation is authentic, complete, and meets the university’s requirements. Specifically, verification usually includes checking:

  1. Legitimacy of the source – Ensuring the document comes from an official and appropriate source (e.g., a licensed doctor, hospital, court, or military authority).
  2. Completeness of the document – Making sure all required information is present, such as dates of absence, signatures, credentials, or official letterhead.
  3. Relevance of the dates – Confirming that the dates on the documentation match the days the student missed class.
  4. Clarity and readability – Making sure the document can be clearly interpreted and isn’t altered or incomplete.

Steps for Submitting Documentation:

  1. Gather, review, and ensure that your documentation meets the Absence Verification Process criteria. This helps prevent verification process delays.
  2. Email your complete documentation with the subject line: "Absence Verification Process" to DisabilityServices@uncfsu.edu.

Next Steps:

  1. Your documentation will enter the absence verification process and be reviewed by the CPDC Absence Verification Process team.
  2. Once fully verified, the CPDC will send a confidential email notification to both you and your professors regarding the documented absence.

If verification cannot be completed based on the documentation submitted, you will receive an email notification outlining what is missing, any requests for additional documentation, and/or a reiteration of the documentation guidelines necessary for the FSU CPDC absence verification process.
